Let's say the unwind rule for missing a mando is in effect.
If I miss the mando, can I call Optional Rethrow rather than unwind?

There is no "Unwind rule" for missing a mando since the 2002 rules update. But yes, you can call an Optional Rethrow if you miss a mando and would prefer to throw from your previous lie/retee rather than play from the Drop Zone. It's a 1-throw penalty either way and you still have to pass the mando on the correct side on your next or future throw.

Unwinding from a mando is not allowed unless the TD gets approval from the PDGA as a rules variance (unlikely since several rules would then be out-of-sync).

No confusion there. The Optional Rethrow rule is just making it clear that it's not a "mulligan" where you just rethrow without penalty but that you count the original throw plus 1-penalty throw. For the missed mando, the rule is stating that you add a 1-throw penalty to your score at the time which would be one if you missed the mando on your tee shot. So you're shooting your third throw from the Mando drop zone.

From the 1997 rulebook:
B. If the disc should pass a mandatory on the incorrect side, the player must throw the disc back past the incorrect side of the mandatory in an unwinding fashion, in order to pass to the correct side.

If you miss the mando tree on the wrong side, your next throw has to go back the same way it missed on the wrong side so you can then throw around the correct side of the mando.
