Hole 12 at charlie vettiner park in louisville, KY. About a 20 foot span in which you are walking over an underground tunnel. At either end of the small tunnel, is a o.b. area. If you are inside the tunnel, between the o.b.'s, are you allowed to play it above the tunnel, or play it where it last was inbounds? No penalty, play above your lie, or penalty, play from last o.b.

Unless marked as OB such as an OB road over a culvert pipe, the playing surface over a tunnel on the course would be played as if the tunnel wasn't under it - no penalty or relocation of the lie.

The rules don't really address layered playing surfaces. There are Q&As that sort of address layered playing surfaces. Bottom line: if it is a tournament or league, you should ask the TD at the player meeting. If it is a casual competitive round, you should agree on ground rules before teeing off. Chuck's approach is reasonable, but it is the TD's call as to what is out of bounds.

What if the disc landed five or six feet into the culvert (assume there's no OB around). Could you play from above the culvert?

No. The culvert is a playing surface that you must attempt to play from unless the TD indicates otherwise in advance. And if you can't play from there (culvert size too small) you have to take an unplayable lie penalty and probably rethrow from your previous lie since it's unlikely you would be able to move up to 5m back on the LOP from your lie in the culvert.
